Your privacy at FORM.
How we use your information, who can see it and the choices you have.
About FORM
Dr Mike McGreary operates FORM as a sole trader. For privacy questions, contact drmikemcgreary@gmail.com. This notice covers adult clients and under-18s receiving selected sport services.
Your information
We collect contact details, relevant health information, goals, scores, reviews and notes from you and your practitioner to provide support and keep appropriate records. For younger clients, we explain privacy in age-appropriate language and involve someone with parental responsibility where appropriate. Parents do not automatically receive private notes. Optional measurements and personal reflections are your choice. Missing essential information may limit the support we can offer.
Why we use it
We use information to fulfil our agreement, run the service safely and meet legal duties. Necessary health information is used to provide confidential psychological healthcare. For coaching outside healthcare, we obtain explicit consent before using sensitive information. You can withdraw that consent; earlier lawful use is unaffected.
Who can see it
Your assigned practitioner sees shared records; other clients cannot. Personal reflections are hidden from the practitioner dashboard, but authorised technical staff may need access. Separate practitioner notes can still be subject to an access request. Other sharing is discussed with you, unless a lawful disclosure is necessary, for example to address serious harm.
Storage and retention
The planned service uses Supabase, Resend, Cloudflare and Google for records, hosting, sign-in emails and correspondence. Some processing takes place overseas; applicable safeguards must be in place. We do not sell your information. We keep information only as long as needed for your support and appropriate professional record-keeping. This depends on the type of information and whether your support is ongoing. You can ask how long your records will be kept.
Your choices
Contact Mike to request access, correction, deletion, a portable copy, or to restrict or object to use. Rights depend on the circumstances; we explain any limits. Requests are normally free and answered within one month, subject to permitted extensions. You can complain to the ICO. Decisions about your support are made by your practitioner, not solely by automation.
